Payment

We accept Visa, MasterCard, Discover, cash, and checks.
We do not bill insurance companies for services however we will provide a SUPER INVOICE with diagnostic and procedure codes for you to submit to your insurance company for reimbursement.

 

Prescription Refills

If you are a new patient and need a refill on a prescription, please note, we will not refill any prescription until you are an established patient.

 

Medical Records

At your initial consultation and yearly thereafter, it is important that you release your most recent medical records to our office.
In compliance with HIPAA rules, we require a signed medical release form each time you request your medical records to be sent from our office. Turnaround time is usually within 3-5 business days.
